Understanding Gynecomastia (Enlarged Male Breasts)

Gynecomastia is a common condition that causes boys' and men's breasts to swell and become larger than normal. It is not excess fat; it is an enlargement of the actual glandular tissue of the breast. The condition can be a source of significant emotional distress and self-consciousness.

What Causes Gynecomastia?

Gynecomastia is most often caused by a hormonal imbalance. Specifically, it's due to an imbalance between the hormones estrogen and testosterone. While estrogen is known as a female hormone, men also produce it in small amounts. If a man's testosterone level is too low compared to his estrogen level, or if his body is overly sensitive to estrogen, it can cause breast tissue to grow.

This imbalance can happen naturally at different life stages:

  • Infancy: Due to estrogen from the mother.
  • Puberty: Due to fluctuating hormone levels. This often resolves on its own.
  • Older Age: As testosterone levels decline with age.

Other causes can include:

  • Certain medications (e.g., some anti-androgens, steroids, and heart medications).
  • Obesity, which can increase estrogen levels.
  • Medical conditions like hypogonadism, kidney failure, or liver disease.
  • Use of recreational drugs or excessive alcohol.

Diagnosis: Glandular Tissue vs. Fat

The first step is a physical exam by a doctor to confirm that the enlargement is due to glandular tissue and not just fat (a condition called pseudogynecomastia). The doctor will also review your medical history and medications. A comprehensive hormone panel is often ordered to check for any underlying hormonal imbalances.

Treatment Options

Treatment depends on the cause and severity of the condition.

  • Observation: If gynecomastia occurs during puberty, it often goes away on its own within a few months to two years.
  • Treating the Underlying Cause: If the gynecomastia is caused by a medical condition or medication, addressing that issue may resolve the breast enlargement.
  • Medical Therapy: In some cases, medications that block the effects of estrogen (like Tamoxifen or Raloxifene) may be prescribed.
  • Surgical Treatment: For persistent or severe gynecomastia, surgery is the most effective option. This can involve liposuction to remove excess fat and/or mastectomy (excision) to remove the glandular breast tissue.
