Understanding Hypogonadism (Low Testosterone)

Hypogonadism is the medical term for a condition in which the body doesn't produce enough testosterone, the primary male sex hormone. This deficiency can lead to a wide range of symptoms that affect a man's quality of life, energy, and overall health.

Types of Hypogonadism

There are two main types:

  • Primary Hypogonadism: This means the problem originates in the testicles. The brain is sending signals to produce testosterone, but the testicles are unable to respond adequately. This can be caused by injury, genetic conditions (like Klinefelter syndrome), or other testicular issues.
  • Secondary Hypogonadism: This means the problem originates in the brain (specifically the hypothalamus or pituitary gland). The testicles are healthy, but they aren't receiving the proper hormonal signals (LH and FSH) from the brain to tell them to produce testosterone. This is the more common type and can be caused by obesity, type 2 diabetes, chronic stress, or other conditions.

Symptoms of Low Testosterone

The symptoms of hypogonadism are often gradual and can be mistaken for normal aging. They include:

  • Persistent fatigue and low energy
  • Reduced sex drive (low libido)
  • Erectile dysfunction
  • Depressed mood or irritability
  • Difficulty concentrating and "brain fog"
  • Loss of muscle mass and increased body fat

A diagnosis of hypogonadism requires blood tests to confirm low testosterone levels, ideally with at least two separate morning measurements.
