- SHBG (Sex Hormone-Binding Globulin): This is a protein that binds tightly to testosterone, making it inactive. Think of it as a taxi that keeps testosterone "occupied." High levels of SHBG can mean that even if your Total Testosterone is normal, very little of it is available for your body to use.
- Free Testosterone: This is the "unbound," active testosterone. It's the hormone that can actually enter your cells to build muscle, boost libido, and improve energy. This is the number that most closely correlates with how you feel.
A man can have a "normal" Total Testosterone level but still suffer from symptoms of low T if his SHBG is high and his Free Testosterone is low.
